Job summary

Bridge Home Health & Hospice is seeking a Clinical Manager to coordinate and supervise patient care in a home health setting. You will review referrals, assign clinicians, and ensure services are delivered effectively while maintaining high-quality patient outcomes.

You will lead clinical staff, participate in case conferences, and support staff development, policy implementation, and community outreach as needed. This role requires a licensed RN with management experience and CPR certification.

Qualifications

• Registered nurse licensed to practice in the state.
• Minimum two years of management or supervisory experience in healthcare, preferably home care.
• CPR certification and strong written/verbal communication.

Responsibilities

• Coordinate patient care by reviewing referrals and assigning clinicians.
• Supervise clinicians, monitor services, and ensure quality home care delivery.
• Review cases, guide staff, and be available during operating hours.
• Attend case conferences and communicate findings to leadership.
• Assist in hiring, orientation, and ongoing staff education.
• Contribute to policy development and organizational planning.
• Promote customer service and community outreach.
