Qualifications

• RN with current California license
• BSN or higher in nursing or health-related field with 3+ years supervisory or administrative experience in hospice/home health
• Demonstrates autonomy, assertiveness, flexibility, and cooperation in performing job responsibilities

Responsibilities

• Coordinate and oversee direct and indirect patient services by clinical personnel.
• Lead interdisciplinary team assessments, planning, implementation and evaluation of patient care.
• Assist Clinical Managers with oversight of clinical teams and workflow.
• Oversee documentation, records and reports for agency activities.
• Participate in on-call rotation and budget administration.
• Ensure compliance with licensure, ACHC Home Care standards and policies.
• Promote hospice referrals within the healthcare community.
