Job summary

Bridge Home Health & Hospice seeks a Director of Patient Care Services to direct hospice or home health clinical operations in California. The role drives goals, quality, and staff performance across the agency.

The DPCS oversees care coordination, budgeting, compliance, and education programs while ensuring ACHC standards are met and referrals are promoted in the healthcare community.

Qualifications

• Registered nurse with current California licensure.
• BSN or higher degree with three years in the last five in hospice/home health, with supervisory experience, or four years in hospice/home health with supervisory experience.

Responsibilities

• Coordinate and oversee all direct and indirect patient services delivered by clinical staff.
• Lead the Interdisciplinary Group (IDG) meetings and coordinate care planning.
• Assist Clinical Managers with oversight of clinical teams.
• Oversee patient records, statistics, and reporting for quality assurance.
• Participate in budgeting, on-call rotation, and staffing considerations.
• Promote compliance with licensure, certification, ACHC standards, and company policies.
