The ideal candidate holds a current nursing license, a Bachelor’s degree in nursing, and 3–5 years of home care nursing experience, with strong skills in coordination and communication.

Qualifications

• Registered nurse with current licensure.
• Bachelor’s degree in nursing from an accredited program.
• 3–5 years of clinical nursing experience, home care preferred.
• Strong understanding of performance improvement, quality assurance, and utilization management.
• Self-directed and able to work with minimal supervision.
• Excellent coordination and communication skills.

Responsibilities

• Implement and monitor the organization’s quality improvement program.
• Coordinate safety, risk management, and infection control monitoring and education.
• Collaborate with nursing supervisors to ensure orientation, competency and inservice education.
• Ensure compliance with Medicare Conditions of Participation and ACHC standards.
• Serve as a clinical resource and assist with patient visits as needed.
• Support community education efforts and maintain positive community relationships.
• Promote professional growth and collaboration within the home care industry.
