Job summary

Anchor Health in Napa/Solano County seeks a Hospice Clinical Liaison to grow patient census and serve as a community ambassador for our hospice program.

The role develops outreach strategies to cultivate referral sources and maintain relationships with physicians, hospitals, SNFs, and other partners. Ideal candidate excels at relationship-building and collaboration throughout the referral-to-admission process.

Qualifications

• LVN, RN or MSW required.
• 2+ years healthcare marketing/sales experience preferred.
• Minimum of 2 years hospice experience highly preferred.
• Strong familiarity with local referral partners: hospitals, physicians, B&C, ALF, RCFE, SNF.
• Excellent written and verbal communication skills.
• Self-directed and able to work independently.
• Consistent, persistent, patient, compassionate interpersonal approach.
• Valid driver’s license, clean driving record, and current auto insurance.

Responsibilities

• Build and maintain relationships with physicians, hospitals, SNF, discharge planners, and community partners.
• Plan and coordinate marketing events, educational sessions, and community luncheons.
• Educate healthcare stakeholders on hospice and palliative care services.
• Support patients, families, and referral partners throughout the referral and admission process.
• Provide hospice informational/ goals of care conversations with patients and families.
• Facilitate initial patient consent process.
• Attend weekly meetings to report successes and growth opportunities.
• Meet or exceed performance expectations set by leadership.

Position Summary

The Hospice Clinical Liaison is responsible for growing patient census while serving as a positive ambassador for our hospice program within the community. This role develops and implements structured outreach strategies to cultivate new referral sources and maintain strong, ongoing relationships with physicians, hospitals, skilled nursing facilities, and other key partners.

The ideal candidate is solutions-oriented, proactive, and excels at relationship-building, follow-through, and collaboration throughout the referral-to-admission process.

• Residential Care Facilities for the Elderly (RCFE)
• Skilled Nursing Facilities (SNF)

• Meet or exceed performance expectations set by the Director of Business Development and Executive Director leadership

Compensation & Benefits
