Job summary

Seaport Hospice in North County San Diego is seeking a compassionate Clinical Liaison to coordinate transitions of care from hospitals and SNFs to hospice services. This role blends clinical assessment, discharge planning, patient education, admissions support, and referral relationship development to ensure smooth continuity of care.

Qualifications

• California Registered Nurse (RN) or Licensed Vocational Nurse (LVN) license is required.
• Experience in home health, hospice, case management, discharge planning, care coordination, or clinical liaison services is preferred.
• Strong communication, relationship-building and patient education skills are essential.
• Ability to work independently while managing multiple priorities and clinical documentation.

Responsibilities

• Develop and maintain relationships with hospitals, SNFs, physicians, and other referral sources.
• Conduct onsite patient assessments following hospice referrals.
• Review physician orders and evaluate clinical needs for referrals.
• Educate patients and families regarding hospice services.
• Coordinate safe transitions from acute or post-acute settings to home-based care.
• Collaborate with physicians, case managers, social workers, and facility staff on discharge plans.
• Ensure physician oversight and required orders for home health and hospice plans of care.
• Communicate referral and clinical information to interdisciplinary care teams.
• Support hospice admissions and reduce avoidable hospital readmissions.
