Qualifications

• Current California LVN license required.
• Minimum 1 year experience in clinical or sales healthcare setting.
• Proven ability to develop and maintain professional referral relationships.
• Excellent communication, organization, and time-management skills.
• Familiarity with EMR systems and computer literacy.
• Valid driver's license and reliable transportation.

Responsibilities

• Establish and grow professional referral relationships with medical communities and facilities.
• Market hospice and home health services to referral partners.
• Serve as primary hospice resource to patients and families.
• Coordinate admissions and care transitions with care teams.
• Read histories, summarize timelines, and explain consents.
• Ensure HIPAA compliance and maintain patient confidentiality.
• Cover Santa Clara County with travel as needed.
