Job summary

Reliance Hospice and Palliative Care in Orange County seeks a proactive Clinical Liaison to build and sustain referral relationships with hospitals, facilities and health care providers. You will educate patients and families about hospice services and coordinate smooth transitions onto hospice care.

The role covers South Orange County up to San Clemente, is full-time and on the road, with a salary up to $90,000 plus bonus and benefits including a 401(k).

Qualifications

• Bachelor's degree in nursing or health care preferred.
• Current LVN license.
• Minimum one year of sales/marketing experience in healthcare.
• One to two years' experience in hospice/palliative marketing.
• Speaks either Spanish and/or Tagalog (preferred).

Responsibilities

• Builds and maintains relationships with referral sources to position Reliance Hospice and Palliative Care as a preferred provider.
• Makes regular calls to secure new referrals and meet census goals with physicians, facilities, case managers, discharge planners and social workers.
• Conducts initial meetings with patients and families to explain hospice services and obtain consents for care.
• Coordinates referral process with admission and intake teams to ensure smooth admissions.
• Maintains positive working relationships with current and potential referral sources.
• Identifies opportunities for expanded or improved services through input in strategic planning.
• Stays informed on community resources to assist referrals when needed.
• Other duties as assigned.
