Job summary

Lorian Health is seeking a Hospice Care Specialist/Community Liaison to grow and manage referral relationships with physicians, hospitals, SNFs, and social workers. You will serve as the face of Lorian Hospice, educate partners, and drive awareness and admissions through strategic outreach.

Qualifications

• Experience in Home Health or Hospice settings within the last 3 years.
• 1–2 years in hospice or palliative care marketing preferred.
• Demonstrated ability to grow and sustain referral relationships.
• Proven track record of meeting or exceeding sales/referral targets.
• Strong presentation and interpersonal skills.
• Ability to manage a dynamic territory and prioritize independently.

Responsibilities

• Develop, manage, and grow referral relationships with physicians, hospitals, SNFs, case managers, social workers, and community partners.
• Represent Lorian Hospice professionally in the community as a Community Liaison / Hospice Care Specialist.
• Execute strategic sales and marketing plans to meet referral and census growth goals.
• Plan, track, and document sales activities and pipeline in the CRM.
• Conduct face-to-face visits, in-services, and educational presentations to raise awareness of hospice services.
• Maintain a strong pipeline and ensure timely follow-up with referral outcomes.
• Collaborate with the interdisciplinary clinical team to ensure smooth transitions of care and high satisfaction.
• Provide patient and family advocacy through hospice eligibility, benefits, and enrollment guidance.
• Document referral activity and outcomes in line with policies.
• Participate in team meetings and ongoing professional development.
