Job summary

Angela Hospice is seeking a Clinical Coordinator/On-Call RN to coordinate and evaluate patient services in a home care setting. You will triage calls, assign admissions to RNs, order medications, and coordinate transfers and discharge planning while providing clinical support to the care team.

Qualifications

• Graduate of an accredited nursing program with active Michigan license.
• CPR certification is required and hospice experience preferred.
• Triage nursing experience is strongly preferred.
• Excellent organization, time management and communication skills.
• Proficient computer skills and ability to work with minimal supervision.
• Ability to lift/transfer patients and maintain safety.

Responsibilities

• Coordinate, implement and evaluate patient services delivery.
• Chart patient/family calls and triage information.
• Assign admissions to RN per Patient Access Center needs.
• Order medications and notify RN Case Manager; document details.
• Arrange transfers and tuck-in visits; manage discharge prep.
• Follow up on overnight/weekend calls; coordinate home care supplies.
• Communicate with patients, families via phone, secure messaging or telehealth.
• Maintain admissions/deaths/changes/tracking daily with location data.
• Provide clinical support to the team delivering patient care.
