Job summary

Gentiva Hospice is seeking a Chaplain to provide spiritual and emotional support to patients and families in a flexible PRN schedule. You will collaborate with the interdisciplinary team, assess needs, and develop individualized care plans in alignment with patients' beliefs.

You will conduct visits, offer blessings and rituals as requested, and support families through grief and bereavement, while maintaining accurate documentation and contributing to a compassionate home-based care program.

Qualifications

• Bachelor's degree in theology, religion, human services, counseling, psychology, or sociology (or as required by state-specific regulations)
• A minimum of one ACPE-accredited Clinical Pastoral Education (CPE) unit

Responsibilities

• Provide spiritual and emotional support to patients, families, and caregivers in line with beliefs and values.
• Offer non-denominational, inclusive care respecting all faiths and cultures.
• Collaborate with the interdisciplinary team in patient care conferences.
• Assess spiritual needs and develop individualized care plans.
• Conduct visits, prayers, blessings, and rituals as requested.
• Assist families during grief and bereavement; provide counseling.
