Job summary

Heart to Heart Hospice in Texas seeks a Chaplain - PRN to provide spiritual care to patients and families, assessing needs across faith traditions and offering crisis counseling. You will participate in IDG meetings, coordinate bereavement support, and ensure compliance with hospice regulations.

Advanced pastoral knowledge is required; pathways include formal theological education, CPE, board certification, or ordained ministry with ministry experience.

Qualifications

• Formal theological education or equivalent pathways accepted.
• CPE experience or board certification accepted as alternate routes.
• Ordination plus 3+ years pastoral ministry with bereavement experience also valid.

Responsibilities

• Independently manage assigned hospice caseload and scheduling.
• Conduct spiritual assessments for patients and families across faiths.
• Provide pastoral counseling and crisis intervention in real time.
• Participate in Interdisciplinary Group (IDG) with care team.
• Manage bereavement contacts after patient death per professional assessment.
• Collaborate to resolve spiritual, pastoral, and ethical issues in care.
• Maintain compliance with regulations and internal policies.
• Perform other duties as assigned.
