Job summary

Continuum Care Hospice in Ohio seeks a compassionate Chaplain (PRN) to provide spiritual care and emotional support within home-based and facility settings. You will honor patients' beliefs and collaborate with the interdisciplinary team to deliver holistic end-of-life care.

The role requires strong communication, crisis-support abilities, and the capacity to work independently within a mission-driven hospice environment. Travel to patient sites and flexible scheduling are involved.

Qualifications

  • Bachelor’s or Master’s in Divinity, Theology, Pastoral Studies, Counseling preferred.
  • Ordination, endorsement, or certification through a faith body preferred.
  • Experience in hospice, chaplaincy, or pastoral care preferred.
  • Ability to work with diverse spiritual and cultural backgrounds.
  • Strong communication and crisis-support skills.

Responsibilities

  • Provide spiritual and emotional support to patients and families in homes, facilities, or hospice settings.
  • Offer grief counseling, crisis support, and end-of-life spiritual care.
  • Respect and support diverse cultural, religious, and non-religious belief systems.
  • Conduct spiritual assessments and document all visits per regulatory and agency standards.
  • Participate in interdisciplinary team meetings to contribute to patient care planning.
  • Facilitate memorial services, rituals, prayer, or requested spiritual practices.
  • Provide bereavement support to families before and after loss.
  • Serve as a liaison with community clergy or spiritual leaders when requested.
  • Maintain strict confidentiality and follow ethical guidelines for chaplaincy.
  • Assist with bereavement program development and follow-up.

Chaplain- PRN

We are seeking a caring and professionalChaplain to join our interdisciplinary team and offer meaningful spiritual and emotional support during life’s most vulnerable moments. Continuum Care Hospice is a mission‑driven hospice agency dedicated to providing compassionate end‑of‑life care for patients and their families. We believe in supporting the physical, emotional, and spiritual well-being of those we serve.

TheChaplain provides spiritual counseling, emotional support, and grief resources to hospice patients and their loved ones. This role ensures that each patient’s beliefs, values, and cultural backgrounds are honored. The chaplain is a vital member of the hospice care team and contributes to holistic, patient‑centered care.

Key Responsibilities
  • Provide spiritual and emotional support to patients and families in homes, facilities, or hospice settings

  • Strong communication, compassion, and crisis‑support skills
  • Valid driver’s license and reliable transportation for home visits
  • Ability to work independently while being part of a collaborative interdisciplinary team
