Job summary

Gentiva Hospice seeks a compassionate Chaplain to provide non-denominational spiritual and emotional support to patients and families at end-of-life. As part of an interdisciplinary team, you will help design individualized spiritual care plans and attend care conferences.

You will conduct visits, prayers, and rituals, support grieving families, and build connections with faith communities while documenting assessments and outcomes to ensure consistent, respectful care.

Qualifications

• Bachelor’s degree or state-regulatory equivalent required.
• Minimum one unit of ACPE-accredited CPE must be completed.
• Accredited degrees preferred; AVPO approval required if non-accredited.

Responsibilities

• Provide spiritual and emotional support to patients, families, and caregivers in alignment with beliefs and values.
• Offer non-denominational, inclusive care respecting all faiths and cultures.
• Work as part of the interdisciplinary team, attending patient care conferences.
• Assess spiritual needs and develop individualized care plans.
• Conduct visits, prayers, blessings, and rituals as requested.
• Support families during grief, loss, or transition and provide bereavement counseling.
• Build relationships with local faith leaders and community resources.
• Maintain timely documentation of spiritual assessments, interventions, and outcomes.
