Job summary

Professional Healthcare Resources seeks a Hospice Chaplin to provide spiritual support to patients and families as part of the Hospice Team. The Chaplain assesses spiritual needs and offers counseling while respecting diverse beliefs within the care plan.

Key duties include direct ministry after admission, development of individualized care plans, documentation of assessments, and collaboration with the interdisciplinary team to address ethical issues and bereavement needs.

Qualifications

• Must have a degree or certification that supports the ability to perform in this role.
• Two (2) years pastoral counseling experience.
• At least one-year (1) counseling terminally ill patients and their families in a hospice or similar setting, preferred.

Responsibilities

• Provide spiritual/pastoral counseling in line with the plan of care.
• Direct ministry and guidance to patients and families after admission.
• Develop an individualized spiritual/pastoral plan of care.
• Offer discussions across beliefs to patients and families.
• Document assessments and care plans; record visits.
• Collaborate with other professionals to resolve spiritual/ethical issues.
• Work with hospice team to assess family responses to care.
• Provide bereavement visits to families before death and during bereavement period.
• Meet with staff regarding spiritual/pastoral issues affecting performance.
