Job summary

Circle of Life Hospice & Palliative Care seeks a compassionate PRN Hospice Chaplain to provide spiritual and emotional support to patients and families. The chaplain will join the interdisciplinary hospice team, delivering care that respects beliefs, values, culture, and personal wishes.

Responsibilities include spiritual assessments, home and facility visits, prayer and counseling, and collaboration with nurses, social workers, and physicians. Travel and HIPAA compliance are expected.

• Ability to work independently with a flexible PRN schedule.

Responsibilities

• Provide spiritual and emotional support to hospice patients and families.
• Conduct spiritual assessments and identify patient and family spiritual needs.
• Visit patients in homes, facilities, and other care settings.
• Provide prayer, counseling, spiritual guidance, scripture reading, or other services as requested.
• Respect and support patients of all faiths and backgrounds.
• Provide support during times of crisis, grief, and end-of-life transition.
• Assist with advance care planning and end-of-life concerns.
• Provide bereavement and grief support when appropriate.
• Participate in interdisciplinary team meetings and communicate needs.
