Job summary

Gentiva Hospice seeks a chaplain to provide spiritual and emotional support to patients and families in alignment with their beliefs. You will participate in patient care conferences and collaborate with the care team to develop individualized spiritual plans.

You will conduct visits, prayers, and rituals, assist families through grief, and build partnerships with faith leaders to meet diverse spiritual needs. Documentation is required.

Qualifications

• Bachelor's degree in theology, religion, human services, counseling, psychology, or sociology (or as required by state-specific regulations)
• Minimum of one ACPE-accredited CPE unit; degrees from non-accredited institutions require AVPO approval prior to hire.

Responsibilities

• Provide spiritual and emotional support to patients, families, and caregivers in alignment with their beliefs and values.
• Offer non-denominational, inclusive care that respects all faiths, spiritual traditions, and cultural backgrounds.
• Serve as an integral member of the interdisciplinary team, attending patient care conferences and collaborating with nurses, social workers, and medical staff.
• Assess spiritual needs and develop individualized care plans.
• Conduct visits, prayers, blessings, and rituals as requested by patients or families.
• Assist families during grief or loss with bereavement support and counseling.
• Build relationships with local faith leaders and community resources to support spiritual needs.
• Maintain timely documentation of spiritual assessments, interventions, and outcomes.

Education

Bachelor's degree in theology, religion, human services, counseling, psychology, or sociology

ACPE-accredited Clinical Pastoral Education (CPE)
