Job summary

The Care Team Hospice is looking for a compassionate CNA to provide personal care and comfort-focused support to hospice patients in homes and facility settings. You will work with an interdisciplinary team to ensure dignity, compassion, and quality of life throughout the hospice journey.

Standard hours are Monday–Friday, 8:00 AM–4:30 PM, with a territory radius of up to 1 hour around Mishawaka, IN. Benefits include tuition reimbursement, PTO, 401(k), HSA eligibility, and more.

Qualifications

• Active CNA certification is required.
• Hospice aide experience preferred.
• Experience using HCHB (Homecare Homebase) preferred.

• Support patients and families with compassion and respect
• Document care provided accurately and timely in the EMR HCHB (Homecare Homebase)

Field Role Requirements

• Active driver’s license
• Ability to pass a criminal background check
• Ability to pass a Motor Vehicle Record (MVR) check
