Job summary

The Care Team Home Health & Hospice in Mishawaka, IN is seeking a Certified Nursing Assistant (CNA) for a day-time role in a hospice setting. No night shifts, no weekends, with a tuition reimbursement option for LPN/RN paths.

The standard schedule is Monday through Friday, 8:00 AM to 4:30 PM, within up to a 1-hour radius around Mishawaka. Key duties include providing personal care, observing patient conditions, supporting families with compassion, and accurately documenting care in the EMR HCHB.

Qualifications

• Active CNA certification required.
• Hospice aide experience preferred.
• Experience using HCHB (Homecare Homebase) preferred.

Field Role Requirements

• Active driver’s license
• Ability to pass a criminal background check
• Ability to pass a Motor Vehicle Record (MVR) check
