Job summary

The Care Team Hospice is seeking a compassionate Certified Nursing Assistant (CNA) to join our growing team. This role focuses on personal care, comfort, and dignity for hospice patients in home and facility settings.

You will work 2nd shift, 2p-10p, collaborating with an interdisciplinary team to support families through difficult moments and promote quality of life. CNA certification and hospice experience preferred.

Qualifications

• Active CNA certification required.
• Previous CNA experience in hospice/home health/long-term care preferred.

Responsibilities

• Provide personal care, bathing, grooming, hygiene assistance, and comfort measures per care plan.
• Observe, document, and report changes in patient condition to the supervising nurse.
• Support patients and families with compassion, dignity, and respect while promoting comfort and quality of life.
• Assist with mobility, positioning, and activities of daily living in a safe care environment.
• Document all patient care in the electronic medical record (Homecare Homebase).

Key Responsibilities

• Provide personal care, bathing, grooming, hygiene assistance, and comfort measures in accordance with each patient's individualized plan of care.
• Observe, document, and promptly report changes in patient condition to the supervising nurse.
• Support patients and families with compassion, dignity, and respect while promoting comfort and quality of life.
• Assist with mobility, positioning, and activities of daily living while maintaining a safe care environment.
• Complete accurate and timely documentation of all patient care provided within the electronic medical record (Homecare Homebase).

Qualifications

• Active Certified Nursing Assistant (CNA) certification
• Previous CNA experience required; hospice, home health, long-term care, or skilled nursing experience preferred.

Field Role Requirements

• Valid driver's license, reliable transportation, and current automobile insurance.
• Ability to pass a criminal background check and Motor Vehicle Record (MVR) screening.
