Job summary

The Care Team Hospice is seeking a compassionate and dependable Certified Nursing Assistant (CNA) to join our growing team. This role provides hands-on personal care and support to hospice patients in their homes and care facilities, helping maintain dignity, comfort, and quality of life.

You will work with an experienced interdisciplinary team, document patient care in the EMR, and assist with mobility and daily living activities while upholding a safe, respectful environment.

Qualifications

• Active CNA certification required.
• Prior CNA experience; hospice/home health/long-term care preferred.

Responsibilities

• Provide personal care, bathing, grooming, hygiene assistance, and comfort measures per each plan of care.
• Observe, document, and report changes in patient condition to supervising nurse.
• Support patients and families with compassion, dignity, and respect while promoting comfort and quality of life.
• Assist with mobility, positioning, and activities of daily living while maintaining safety.
• Complete accurate documentation of all patient care in the EMR (Homecare Homebase).

Key Responsibilities

• Provide personal care, bathing, grooming, hygiene assistance, and comfort measures in accordance with each patient's individualized plan of care.
• Observe, document, and promptly report changes in patient condition to the supervising nurse.
• Support patients and families with compassion, dignity, and respect while promoting comfort and quality of life.
• Assist with mobility, positioning, and activities of daily living while maintaining a safe care environment.
• Complete accurate and timely documentation of all patient care provided within the electronic medical record (Homecare Homebase).

Qualifications

• Active Certified Nursing Assistant (CNA) certification
• Previous CNA experience required; hospice, home health, long-term care, or skilled nursing experience preferred.

Field Role Requirements

• Valid driver's license, reliable transportation, and current automobile insurance.
• Ability to pass a criminal background check and Motor Vehicle Record (MVR) screening.
