Job summary

The Care Team Hospice in Mishawaka, IN is seeking a compassionate Certified Nursing Assistant (CNA) to provide personal care and comfort-focused support to hospice patients in homes and facility settings. You will work with an interdisciplinary team to ensure dignity and quality of life throughout the hospice journey.

Standard hours are Monday–Friday, 8:00 AM–4:30 PM, with up to a 1-hour radius around Mishawaka. CPR, TB testing, background checks, and MVR checks are required.

Qualifications

• Active CNA certification.
• Hospice aide experience preferred.
• Experience using HCHB (Homecare Homebase) preferred.

Responsibilities

• Provide personal care, hygiene, and comfort measures.
• Observe and report changes in patient condition.
• Support patients and families with compassion and respect.
• Document care provided accurately and timely in the EMR HCHB (Homecare Homebase).

The Care Team Hospice is seeking a CNA to provide personal care and comfort-focused support to hospice patients in their homes and facility settings while working alongside our interdisciplinary team to ensure dignity, compassion, and quality of life throughout the hospice journey.

Schedule, Hours & Territory

• Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM

Field Role Requirements

• Active driver’s license
• Ability to pass a criminal background check
• Ability to pass a Motor Vehicle Record (MVR) check
