Job summary

Residential Home Health and Hospice seeks a Certified Nursing Assistant to provide compassionate hospice care in patients' homes within the Elgin, IL area. A $5,000 sign-on bonus is available.

You will deliver patient-centered care, assist with ADLs, and document care in the EMR while traveling to patient locations. Qualifications

• Active Certified Nursing Assistant (CNA) certificate in the state you work.
• High School Diploma or certificate.
• Minimum one year healthcare experience; Hospice experience preferred.
• Current driver’s license and ability to drive ~20% of the day.

Responsibilities

• Deliver patient-centered care in a hospice setting with an interdisciplinary team.
• Assist with activities of daily living (ADLs) and observe changes.
• Reassure and guide patients and caregivers on the care plan.
• Document care accurately in the EMR and respond to patient needs.
• Work independently, but collaborate when needed; travel to patient homes.

• Provide hands-on care to patients, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support comfort and quality of life
• Support and guide patients and caregivers by reinforcing the care plan, offering emotional support, and helping them understand what to expect during the hospice journey
• Respond promptly to patient needs with empathy and attentiveness and accurately document care provided in the EMR
• Work independently, but never alone
• Travel to patient homes within assigned territory
