Job summary

Residential Home Health and Hospice is seeking a Certified Nursing Assistant to provide in-home care for patients in Lake Forest and surrounding areas. You will work within a hospice/home health team to deliver compassionate care, support ADLs, and educate patients and caregivers.

You’ll drive to patient homes, document in the EMR, and contribute to a high-quality care plan that enhances safety, comfort, and independence for patients in their own homes.

Qualifications

• Active Certified Nursing Assistant (CNA) certificate in the state you work
• High School Diploma or certificate
• Minimum one year healthcare experience
• Current driver’s license and willingness to drive ~20% of day

Responsibilities

• Deliver compassionate, patient-centered care in a hospice/home health setting
• Provide hands-on care in patients' homes and assist with ADLs
• Educate patients and caregivers on care plans and routines
• Communicate changes and document accurately in the EMR
• Work independently while collaborating with the care team

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• Provide hands-on care in the home, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support patient safety and independence
• Educate and support patients and caregivers by reinforcing care plans, promoting healthy routines, and encouraging confidence with daily care tasks
• Respond promptly to patient needs, communicate effectively with the care team, and maintain accurate, timely documentation in the EMR
• Work independently, but never alone
• Travel to patient homes within assigned territory
