Job summary

Residential Home Health and Hospice seeks a dedicated Certified Nursing Assistant to provide compassionate hospice care in patients’ homes around Springfield, IL. You will support ADLs, communicate patient needs to the care team, and document care in the EMR.

The role requires travel to patient locations and a commitment to comfort and dignity. We value autonomy, ongoing learning, and collaborative teamwork.

Qualifications

• Active CNA certificate valid in your work state.
• High School Diploma or equivalent.
• Minimum one year healthcare experience; Hospice preferred.

Responsibilities

• Deliver patient-centered care in a hospice home setting.
• Assist with ADLs and observe changes in condition.
• Reinforce care plans and provide emotional support to patients and families.
• Document care via EMR accurately and promptly.
• Work independently while collaborating with the interdisciplinary team.

We are hiring a Certified Nursing Assistant for a Hospice position providing care to patients in their homes in the Springfield, IL area.

What You’ll Do:

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• Provide hands-on care to patients, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support comfort and quality of life
• Support and guide patients and caregivers by reinforcing the care plan, offering emotional support, and helping them understand what to expect during the hospice journey
• Respond promptly to patient needs with empathy and attentiveness and accurately document care provided in the EMR
• Work independently, but never alone
• Travel to patient homes within assigned territory

CNA Qualifications

• Active Certified Nursing Assistant (CNA) certificate in the state you work
• High School Diploma or certificate
• Minimum one year healthcare experience, Hospice experience preferred
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
