Job summary

Residential Home Health and Hospice in Novi is seeking a compassionate Certified Nursing Assistant (CNA) to provide in-home hospice care. You’ll work with an interdisciplinary team to support comfort, dignity, and quality of life while delivering hands-on ADL assistance.

Qualifications

• Active CNA certificate in the state you work.
• Minimum one year healthcare experience; hospice experience preferred.
• Due to field-based nature, a valid driver’s license is required.

What You’ll Do:

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• Provide hands-on care to patients, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support comfort and quality of life
• Support and guide patients and caregivers by reinforcing the care plan, offering emotional support, and helping them understand what to expect during the hospice journey
• Respond promptly to patient needs with empathy and attentiveness and accurately document care provided in the EMR

• Work independently, but never alone
• Travel to patient homes within assigned territory
