Residential Home Health and Residential Hospice is seeking a compassionate Hospice Certified Nursing Assistant to join our care team. The CNA will provide direct patient care per care plans and collaborate with a multidisciplinary team in a caring, patient-focused environment.

The role requires a state CNA certification, a high school diploma or equivalent, and a valid driver's license with the ability to travel about 20% of the day to patient locations.

Employment Type: Full‑time Employee – 40 hours per week

Overview

Residential Home Health and Hospice is seeking a compassionate Hospice Certified Nursing Assistant to join our care team. The CNA will collaborate with colleagues to manage and deliver patient care plans in a supportive, team‑based environment.

Responsibilities

• Provide direct patient care and support as outlined in individual care plans.
