Job summary

Graham Healthcare Group is seeking a Certified Nursing Assistant to provide compassionate hospice care in patients' homes around West Bloomfield, MI. You will work with an interdisciplinary team to support comfort, dignity, and quality of life for patients in a home setting.

The ideal candidate holds an active CNA certificate in Michigan, a high school diploma or equivalent, and at least one year of healthcare experience (hospice experience preferred).

Qualifications

• Active Certified Nursing Assistant (CNA) certificate in the state you work
• High School Diploma or certificate
• Minimum one year healthcare experience; Hospice experience preferred
• Valid driver’s license and ability to drive to patient locations (~20% of workday)

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team
• Provide hands-on care to patients and assist with activities of daily living
• Support patients and caregivers by reinforcing the care plan and offering emotional support
• Document care provided accurately in the EMR and respond promptly to patient needs
• Travel to patient homes within assigned territory and work independently, but not alone

What You'll Do:

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• Provide hands-on care to patients, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support comfort and quality of life
• Support and guide patients and caregivers by reinforcing the care plan, offering emotional support, and helping them understand what to expect during the hospice journey
• Respond promptly to patient needs with empathy and attentiveness and accurately document care provided in the EMR
• Work independently, but never alone
• Travel to patient homes within assigned territory
