Job summary

Residential Home Health and Hospice is seeking a Certified Nursing Assistant (CNA) to provide in-home care in Clinton Township, MI and surrounding areas. The CNA will perform ADLs, monitor health status, and support patients and families while traveling to homes in the assigned territory.

The role requires an active CNA in the state, a high school diploma, and at least one year of healthcare experience. A valid driver’s license and ability to travel approximately 20% of the workday are required.

Qualifications

• Active Certified Nursing Assistant (CNA) certificate required in the state you work.
• High school diploma or equivalent.
• Minimum one year healthcare experience preferred.

Responsibilities

• Deliver compassionate, patient‑centered care in patients' homes.

• Work independently in the home setting while staying connected with the team.

What You'll Do:

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• Provide hand‑on care in the home, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support patient safety and independence
• Educate and support patients and caregivers by reinforcing care plans, promoting healthy routines, and encouraging confidence with daily care tasks
• Respond promptly to patient needs, communicate effectively with the care team, and maintain accurate, timely documentation in the EMR
• Work independently, but never alone
• Travel to patient homes within assigned territory

CNA Qualifications
