Job summary

Residential Home Health and Hospice is seeking a full-time Certified Nursing Assistant to provide compassionate in-home care in the Naperville area. You will work with an interdisciplinary team, deliver ADL assistance, and document patient status in the EMR.

Qualifications

• Active Certified Nursing Assistant (CNA) certificate in the state you work.
• High School Diploma or certificate
• Minimum one year healthcare experience
• Valid driver’s license and ability to drive ~20% of the time

Responsibilities

• Deliver compassionate, patient-centered care in a home health/hospice setting.
• Assist with activities of daily living (ADLs) and monitor patient condition.
• Educate patients and caregivers on care plans and routines.
• Document accurately in the EMR and communicate changes to the care team.

What You’ll Do:

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• Provide hands-on care in the home, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support patient safety and independence
• Educate and support patients and caregivers by reinforcing care plans, promoting healthy routines, and encouraging confidence with daily care tasks
• Respond promptly to patient needs, communicate effectively with the care team, and maintain accurate, timely documentation in the EMR
• Work independently, but never alone
• Travel to patient homes within assigned territory
