Job summary

Residential Home Health and Residential Hospice in St. Clair Shores, MI is hiring a Hospice Certified Nursing Assistant to deliver compassionate, patient-centered care in patients' homes.

You will work with an interdisciplinary team to support comfort, dignity, and quality of life. This role requires an active CNA certificate, a high school diploma, and a valid driver's license with the ability to travel; hospice experience is preferred.

Qualifications

• Active CNA certificate in the state you work.
• High School Diploma or certificate.
• Minimum one year healthcare experience; Hospice experience preferred.
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life.
• Provide hands-on care to patients, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support comfort and quality of life.
• Support and guide patients and caregivers by reinforcing the care plan, offering emotional support, and helping them understand what to expect during the hospice journey.
• Respond promptly to patient needs with empathy and attentiveness and accurately document care provided in the EMR.
• Work independently, but never alone.
• Travel to patient homes within assigned territory.
