Qualifications

• Graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience in home care.
• Must understand federal, state, and local laws governing hospice operations.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Represent the branch in external transition activities with patients, families, physicians, hospitals, and care teams to promote Enhabit services.
• Develop patient-centered transition plans by integrating clinical guidelines and metrics.
