Job summary

St. Croix Hospice is seeking a Care Transition Coordinator to drive admissions and marketing activities in the Minnesota region.

The role focuses on educating referral sources, coordinating care transitions, and working closely with the Director to meet budgeted volumes. Ideal candidates will hold an associate degree in Marketing, Business Administration, or Healthcare and demonstrate strong communication, negotiation, and organizational skills.

Qualifications

• Associate’s degrees in Marketing, Business Administration, Healthcare, or related field.
• Ability to pass DHS background study.
• Strong written and verbal communication and negotiation skills.

Responsibilities

• Meet admission and call volume targets.
• Coordinate daily sales and marketing operations including marketing initiatives.
• Build and monitor perceptions of St. Croix Hospice as a high-quality provider.
• Document all sales activity in the CRM per requirements.
• Employ marketing/promotional initiatives to achieve budgeted sales.
• Assist Director in budgeting and marketing allocations and resource monitoring.
