Job summary

St. Croix Hospice is seeking a Care Transition Coordinator in Minnesota to cultivate referral relationships and educate families on hospice care. The role includes coordinating intake to ensure timely referrals and supporting the Director in budgeted marketing efforts.

Responsibilities cover meeting admission targets, CRM documentation, and promoting high-quality care while coordinating across patients, families, facilities, and care teams.

Qualifications

• Associate degree in Marketing, Business Administration, Healthcare, or related field.
• Strong communication, negotiation and public relations skills.
• Ability to pass DHS background study.
• Autonomy, organization, assertiveness, flexibility and cooperation.

Responsibilities

• Meet admission and call volume targets.
• Coordinate daily sales and marketing operations and marketing initiatives.
• Build and monitor customer perceptions of St. Croix Hospice as a high-quality provider.
• Document all sales activity in the CRM per requirements.
• Employ marketing and promotional initiatives to reach budgeted sales volumes.
• Maintain knowledge of markets, payers, referral sources, and competitors.
• Enhance communication between patient, family, caregiver, facility, physician team, and vendors.
• Educate patient, family, and caregiver on hospice and informed decision making.
• Assist leadership in budgeting and marketing allocations.
• Monitor resources usage according to budgets.
• Conduct market assessments and support a comprehensive marketing plan.
• Coordinate marketing events, vendor fairs and exhibits in territories.
• Support Care Transition Team training and mentoring.
• Orient new staff in the marketing team.
• Provide leadership in strategic planning to address customer needs.
• Share knowledge of marketing with organization personnel.
• Guide customers to community resources when services are not provided.
• Monitor cost-effectiveness of marketing efforts.
• Perform other duties as assigned.
