Job summary

VitalCaring Group is seeking a Care Transition Coordinator – Hospice to operate in Seminole, targeting the Shawnee territory and Harrah. You will connect patients, families, and partners, coordinating transitions into home health or hospice services with a focus on referral growth.

The role blends healthcare sales, care coordination, and partner development in a fast-paced, field-based environment. A Bachelor’s degree and healthcare sales experience are preferred.

Qualifications

• Bachelor’s degree or equivalent experience in a related field.
• Minimum of two years in healthcare or related sales.
• Strong communication, relationship-building, and influencing skills.
• Ability to manage multiple priorities in a fast-paced field-based environment.
• Proficiency with Microsoft Office and CRM systems.
• Demonstrated ability to work independently and drive results.
• Valid driver’s license and reliable transportation.

Responsibilities

• Serve as the primary liaison between referral sources, patients, caregivers, and clinical teams to ensure seamless care transitions.
• Execute focused sales calls including planning, relationship-building, needs discovery, and closing.
• Build and maintain strong relationships with referral partners through consistent communication and service excellence.
• Develop strategies to grow market share and drive referral volume.
• Track, manage, and document sales activity across referral sources using CRM tools.
