Job summary

VITAS Healthcare in Houston seeks a Team Manager to lead the Patient Care Team, supervise and coordinate an interdisciplinary staff, and ensure continuity of care from admission to discharge or bereavement.

You will advocate for patients, collaborate with social services and community providers, and manage staffing, documentation, and quality-improvement activities using the Vx system.

Qualifications

• Bachelor's degree preferred; RN license required in the state.
• Two years supervisory experience in healthcare.
• Knowledge of primary medical and nursing care and palliative care.
• Familiarity with interdisciplinary team dynamics.
• Proficiency with VITAS Vx system.

Responsibilities

• Directs patient care services for the team and ensures integrated care.
• Reviews admissions, assessments and discharges for eligibility.
• Collaborates with physicians on palliative care measures.
• Ensures accurate data and documentation for recertification decisions.
• Manages team scheduling, payroll and territory assignments.
• Participates in on-call rotation and quality improvement.
