Qualifications

• RN license in the state of California.

Responsibilities

• Directs and coordinates patient care as part of the interdisciplinary team.
• Ensure continuity of care from admission to discharge or bereavement.
• Review admissions, assessments, and discharges for eligibility and plan of care.
• Collaborate with physicians on pain and symptom management.
• Monitor documentation for timely recertification decisions and care planning.
• Manage team scheduling and on-call rotations.
