Job summary

VITAS Healthcare is seeking a Team Manager to lead the Patient Care Team, supervise staff, and coordinate interdisciplinary care from admission to discharge. The role emphasizes advocacy, documentation, and continuous improvement within hospice operations.

The position requires supervisory experience, nursing knowledge, and proficiency with the Vx system to support quality outcomes and patient/family satisfaction.

Qualifications

• Reliable transportation with license and insurance coverage for driver and passengers.
• Private telephone in home.
• Knowledge of primary medical and nursing care, with working knowledge of oncologic nursing and palliative care.
• Understanding of interpersonal dynamics within an interdisciplinary health care team and bereavement.
• Working knowledge of VITAS information system (Vx) and ability to use management reports.
• Two years supervisory experience or equivalent in a health care organization.

Responsibilities

• Directs all patient care services for the team, ensuring integrated delivery.
• Reviews admissions, history and live discharges for eligibility and appropriateness.
• Collaborates with physicians on palliative care measures for pain and symptom management.
• Ensures accurate data for recertification decisions and timely action.
• Monitors services to ensure adequacy and responsiveness to patient/family needs.
• Oversees staff scheduling and 24/7 coordination of team services.
