Job summary

VITAS Healthcare in Dallas seeks a Team Manager to lead the Patient Care Team, ensuring continuity of care from admission to discharge. You will supervise, coordinate the interdisciplinary team, and advocate for patients and families while maintaining thorough records.

You will partner with the team physician and attending physicians on palliative measures, manage schedules 24/7, and drive quality improvement, regulatory compliance, and cost-effective care within hospice standards.

Qualifications

• Two years supervisory experience in a health care setting.
• Bachelor's degree preferred; RN license required.
• Knowledge of primary medical and nursing care; oncology/palliative care knowledge.
• Familiarity with interdisciplinary teams and patient advocacy.

Responsibilities

• Directs patient care services for the team as integrated components of the care plan.
• Reviews admissions, histories and discharges to ensure eligibility and continuity.
• Collaborates with physicians on palliative care measures for pain and symptom management.
• Manages team schedule and 24/7 coordination for all patients.
• Maintains accurate patient records from intake through discharge.
