Job summary

VITAS Healthcare is seeking a Team Manager to lead the Patient Care Team and supervise the interdisciplinary team, ensuring continuity of care from admission to discharge. You will advocate for patients and coordinate with social services and community providers involved in care.

The role requires strong supervision, knowledge of palliative care, and experience in managing patient records. You will oversee staffing, scheduling, and on-call rotations to ensure quality and compliant care delivery.

Qualifications

• Bachelor's degree preferred.
• Current and valid RN License to practice in the state where the VITAS program is located.

Responsibilities

• Directs patient care services for the team as integrated components of the interdisciplinary plan of care.
• Reviews admissions and discharges to ensure eligibility and appropriateness.
• Collaborates with team physician and attending physician on palliative care measures.
• Monitors data and documentation for recertification decisions and timely actions.
• Manages team schedule and coordinates services 24/7 for all patients on the team.
• Facilitates team meetings and ensures interdisciplinary input into the plan of care.
