Job summary

VITAS seeks a Team Manager to lead the Patient Care Team and coordinate care from admission to discharge, acting as patient advocate and coordinating with social services and community providers. This role ensures continuity of care and documentation across the care spectrum.

The position emphasizes staff supervision, schedule management, on-call responsibilities, and adherence to VITAS policies for high-quality, cost-effective care.

Qualifications

• Two years successful supervisory experience or equivalent in a health care organization.
• Knowledge of the principles and practices of primary medical and nursing care, with at least a working knowledge of oncologic nursing and palliative care.
• Current and valid RN License to practice in the state where the VITAS program is located.

Responsibilities

• Directs all patient care services for the team, including volunteer and bereavement.
• Reviews admissions/assessments to assure eligibility and appropriateness.
• Collaborates with team physicians regarding palliative care measures.
• Assures complete and accurate data and documentation are available to make recertification decisions.
• Monitors team services and staffing to ensure productivity and quality.
• Manages team schedule and coordinates services 24/7.
