Responsibilities

• Oversee quality and safe delivery of hospice services during patient stays.
• Provide coaching and support related to hospice care.
• Collaborate with Medical Director to guide care and ensure compliance.
• Meet or exceed quality indicators for staff and services.
• Manage intake, schedule, and staffing to balance referrals.
• Ensure Start of Care compliance and monitor outcomes.
• Implement field quality improvement programs.
• Participate in on-call rotation and manage on-call calendar.
• Address staffing needs, PTO, call-offs, and productivity reviews.
• Conduct field visits and control costs for supplies and visits per episode.
• Coordinate with billing on authorizations and exceptions.
• Maintain relationships with referral sources.
• Lead monthly IDG meetings and review clinical documentation.
• Address complaints and report staffing issues to leadership.
• Maintain active RN license and contribute to in-service training.
• Uphold core values and professional appearance as company representative.
• Apply agency policies and procedures effectively.
