Job summary

VNS Health is seeking a Hospice Operations Coordinator to support the Hospice Care team. You will serve as the primary administrative liaison, coordinating between clinicians, patients, families, and vendors, and managing scheduling and documentation to ensure timely patient care.

The role requires collaboration with the IDG, Intake team, and Team Manager, while maintaining compliant care records and smooth operations across day-to-day activities.

Qualifications

• Associate degree in healthcare or related field preferred.
• Two years of administrative or customer service experience in healthcare.
• Proficiency in Microsoft Office and EHR systems.
• Bilingual skills preferred.

Responsibilities

• Coordinate team communications and scheduling.
• Manage patient care coordination and scheduling of visits.
• Collaborate with Intake, Team Manager, and IDG on admissions.
• Coordinate DME, pharmacy, and supply orders.
• Prepare, facilitate, and close IDG meetings and ensure documentation.
• Ensure timely completion of patient service documentation.
