Job summary

HopeHealth in Providence, RI, seeks a Care Navigator Registered Nurse (RN) to serve hospitals, nursing homes, physician practices and the community. The role drives hospice and palliative care growth and coordinates transitions to HopeHealth service lines.

The RN will collaborate with the Access Center, document in the EMR, and participate in care coordination meetings while maintaining HIPAA compliance. Candidates should be an experienced, compassionate nurse with RI/MA licensure, BSN

Qualifications

• RN required; BSN preferred; five+ years of nursing experience.
• Dual RN license in RI and MA required.
• Valid driver’s license with insured automobile.

Responsibilities

• Cultivates relationships in hospitals, nursing homes, and community to drive hospice and palliative care growth.
• Presents HopeHealth services to patients, families, and referral partners.
• Documents accurate, timely EMR entries and participates in care coordination meetings.
• Coordinates transitions to appropriate HopeHealth service lines with Access Center collaboration.
