Job summary

VNA Home Health & Hospice in Rhode Island is seeking a Hospice RN to manage a caseload as part of an interdisciplinary team. You will assess, plan, and coordinate care and monitor treatment to promote optimal outcomes in a home-based setting.

The role requires a RI Registered Nurse license, CPR, and a Bachelor's Degree in Nursing (or related field). At least 1 year of clinical nursing experience (hospice preferred) and strong communication skills are essential.

Qualifications

RI Registered Nurse license (required).
Bachelor’s Degree in Nursing or related field strongly preferred.
At least 1 year of clinical nursing experience (hospice preferred).
Current CPR certification.
Valid driver’s license and automobile liability insurance.
Excellent written and verbal communication skills.
Strong clinical judgment and problem-solving abilities.

Responsibilities

Performing comprehensive assessments and developing personalized care plans.
Coordinating care and referrals across disciplines and specialties.
Monitoring patient progress and adjusting care to achieve optimal outcomes.
Communicating with patients, families, and providers to ensure continuity of care.
Educating patients and caregivers on managing conditions at home.
Documenting clinical notes and updates with accuracy and clarity.
Identifying and addressing opportunities for care improvement.
