Job summary
HopeHealth, Inc. is seeking an experienced Palliative Nurse Practitioner in Providence, RI to provide direct patient care, symptom management, and care coordination for seriously ill adults.
The NP will collaborate with referring providers, assess prognosis, and support families in establishing goals of care in a hospice/palliative setting. Required are an accredited master’s degree in nursing, NP certification (Family/Geriatric or Adult), RI/MA licenses and prescriptive authority.
Qualifications
- Master’s degree in Nursing from an accredited program.
- NP certification (Family/Geriatric or Adult) and RI/MA licensure required.
- Active RN/NP licenses in RI and MA; prescriptive authority where applicable.
- DEA license may be required for prescribing controlled substances.
Responsibilities
- Provide direct patient care and pain/symptom management for seriously ill adults.
- Collaborate with physicians and care teams; assess disease trajectory and prognosis.
- Communicate goals of care and coordinate end-of-life options with families.
- Ensure accurate documentation and adherence to policies and HIPAA.
- Serve as a skilled hospice/palliative care consultant and educate staff.
