Job summary

HopeHealth in Providence, RI seeks a Care Navigator RN to be the primary representative in hospitals, nursing homes, physician practices and the community. This full-time role promotes hospice and palliative care growth across Rhode Island and coordinates patient transitions to the appropriate service line.

Ideal candidates hold an RN license (BSN preferred) with 5+ years of nursing experience; hospice/palliative care exposure is a plus.

Qualifications

• RN required; BSN preferred; 5+ years nursing experience.
• Hospice, palliative care experience preferred but not required.
• Dual RN license in RI and MA required.
• Valid driver’s license with an automobile that is insured and in good working order.

Responsibilities

• Cultivates relationships with hospitals, nursing homes, physician offices and the community to drive hospice and palliative care growth.
• Acts as subject matter expert in all HopeHealth services including hospice, palliative care, bereavement and community education.
• Presents hospice and palliative care services to patients, families and referral partners highlighting benefits.
• Completes goals of care conversations, hospice informational sessions and hospice evaluations.
• Works with the Access Center to ensure best-in-class service and appropriate patient transitions.
• Documents accurate, timely EMR entries and participates in care coordination meetings.
• Leverages CRM to identify growth opportunities and records territory activity regularly.
