Qualifications

• Graduate of National League for Nursing accredited school of nursing.
• Minimum of two years of experience in public health, home care, or hospice nursing preferred.
• Registered nurse with current licensure to practice in the state.
• Demonstrates excellent observation, problem solving, verbal and written communications; nursing skills per competency checklist.
• Shows ability to organize and prioritize workload independently.

Responsibilities

• Completes initial, comprehensive and ongoing assessments of patient and family to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Develops a care plan with goals based on nursing diagnosis and involves patient and family.
• Administers medications and treatments as prescribed by the physician.
• Acts as Case Manager coordinating care for assigned caseload.
• Teaches patient and family self-care techniques and health instructions.
