Qualifications

• Active and unencumbered RN license in state of employment.
• Bachelor's degree in Nursing highly preferred.
• 2–3 years of related healthcare experience required.
• Experience in healthcare marketing is a plus.

Responsibilities

• Assist referral sources in identifying patients matching hospice services.
• Explain hospice benefits to patients and families to facilitate understanding.
• Coordinate admission by communicating with hospitals, facilities, and physicians.
• Support the admission process and patient discharge planning from hospital to home.
