Job summary

Mercy Health Home Care and Hospice by Compassus is seeking a Hospice Clinical Liaison with an active RN license to promote Compassus values and to coordinate admissions from hospitals and facilities. You will assess patient needs and collaborate with physicians to ensure appropriate hospice eligibility and support families through the process.

The role requires 2–3 years of related clinical experience; healthcare marketing experience is a plus.

Qualifications

• RN license is required and governs clinical practice.
• Two to three years of related experience are expected.
• Hospital and/or long-term care exposure is preferred.

Responsibilities

• Attend staff meetings, educational programs, and community events as requested.
• Explain hospice services and benefits to patients and families clearly.
• Coordinate admissions from hospital and long-term care sources.
• Establish professional relationships with external stakeholders.
• Assist referral sources in identifying patients matching hospice services.
